Microstomia Orthosis with Vertical Screw Mouth Stretching Device in the Facial Burn

Abstract
Purpose: Patients who have sustained a facial burn often contend with cosmetic and functional problem including microstomia. Many devices have been invented to provide oral stretching. The purpose of this article is to present a vertical screw mouth stretching orthosis with clinically practicing for some cases.

Methods: By using thermoplastic splinting material and a long thread screw, a vertical mouth stretching orthosis can be custom fit. The device can be fabricated for the rehabilitation of a circumferential mouth burn, especially targeting the vertical diameter.

Results: This device was fabricated for two facial burn patients who sustained 18% and 24% TBSA in a flame fire with extensive involvement of the face. They developed facial hypertrophic scar and required pressure facial mask. Both patients had moderate to severe microstomia. This device was designed as a lip based splint adjusting the vertical height of each splint piece by screwing. When using this device, patients gave positive feedback for comfort and ease of use, with increased mouth mobility and range of motion.

Conclusion: This vertical screw microstomia orthosis is easy appliable and convenient to change heightening and economic to be fabricated, however that has some problems which ought to be improved to more effective and comfortable function.
